 Enrollment Steps and Procedures

 A. School of Law
  1. Visit the following link for your specific enrolment schedule: https://tinyurl.com/SOL-Enrollment
  2. Proceed to the office of the School of Law located on the ground floor of the new Graduate School building and submit the following enrolment documents:
  3. Original Transcript of Records (TOR)
  4. Original copy of PSA Birth Certificate / Marriage Contract (for female)
  5. Two (2) reference forms (Kindly download forms here):  SOL Reference Form
  6. Two (2) pcs passport-size colored picture, white background with name tag.

Note: Place the documents in a long brown envelope.

          Indicate the following information on the top left portion of the envelope:

  • Name of Student
  • Program
  • Contact Number
  1. Secure the Advising Slip from the same office.
  2. Proceed to the Registrar’s Office and get your Assessment Form.
  3. Pay your tuition and other school fees at the Cashier’s Office. (The Cashier’s office is open from 8:00 am - 4:00 pm only.)
Claim your Certificate of Registration at the Registrar’s Office.

 B. College - New Students (Incoming First year)

  1. Go to your respective campuses to submit your enrolment documents.
  2. Proceed to the designated registration counters for the evaluation of the following documents:

a.  Original copy of Report Card (Form 138)
b.  Original copy of Certificate of Good Moral Character
c.  Photocopy of PSA Birth Certificate

Note: Place the documents in a long brown envelope.

Indicate the following information on the top left portion of the envelope:

For Bacolor Campus’ enrollees, proceed to the Registrar’s Office and secure your Certificate of Registration.

 

 C. College - Regular Students (Second year - Fifth year levels)

  1. The chairperson shall forward the final list of students with a Letter of Intent (LOI) to the Registrar’s Office on or before June 8, 2026.
  2. The records in charge of the program shall process the enrollment of the students.

 D. College - Irregular Students

  1. The adviser/chairperson shall:

           2.1.  Check the Academic Program Evaluation at the student portal; 
           2.2.  Contact the student for advising, which will be done in the respective unit; and
           2.3.  Submit the assessment form to the dean for approval.

  1. The chairperson will forward the approved forms to the Registrar’s Office.
  2. The personnel in charge of the records shall process the enrolment of irregular students.

 E. College - Shiftees

  1. The student shall secure a "Permit to Shift" Form from the personnel in charge of the records at the Office of the University Registrar
  2. The "Permit to Shift" Form, duly signed by the parent/guardian and a copy of grades from the last semester attended, shall then be submitted to the unit where the student intends to enroll.
  3. The Chairperson shall then evaluate the student's academic records and return the shifting form with remarks approved/disapproved to the Office of the University Registrar.

Note: Students with grades lower than 85.0 or equivalent in all enrolled subjects and who wish to shift to another academic program with a board exam shall not be allowed.

 F. College - Transferees

  1. Submit all enrollment documents together with the notarized undertaking and opt-out form
  2. The personnel in charge of the records of the program shall check, evaluate, and accept the enrollment documents, then issue an assessment form.
  3. The student will then proceed to the Cashier’s Office to settle fees.
  4. Finally, the official receipt must be submitted to the Registrar’s Office to receive the Certificate of Registration.

 G. College - Returnees

  1. Submit a Letter of Intent addressed to the University Registrar, duly signed by the parent/guardian and the former Dean/Director.
  2. The personnel in charge of the records shall issue an Academic Program Evaluation to the student.
  3. The student shall then proceed to the respective unit where he/she wishes to enroll and present a copy of grades for evaluation.
  4. The Dean/ Director shall endorse the student and forward the properly accomplished advising form to the Office of the University Registrar for appropriate action.
  5. The personnel in charge of the records shall process the enrolment of the student.

 H. Basic Education - Junior High School

For NEW Students

  1. The student/parent must submit all the following necessary documents:
  1. Six (6) pcs 2x2 picture, white background with nametag
  2. Two (2) photocopies of the PSA Birth certificate
  3. Original and 2 photocopies of F9 (Final Report Card)
  4. Original and 2 photocopies of Good Moral Certificate
  5. Two (2) Long brown envelopes
  6. Accomplished the Pre-Registration Form
  1. The name of the student will be checked by the faculty-in-charge (FIC) if on the approved list.
  2. The FIC will also check the completeness and authenticity of the submitted documents.
  3. Upon verification that all required documents are complete and that the form was properly accomplished, signature/s will be affixed on the pre-assessment slip. Such form will be given to the parent or student.
  4. The student/parent will pay the necessary fees at the Cashier’s Office. An official receipt will be issued to confirm this payment.
  5. The issuance of Certificates of Registration, Class Schedules, sections, and capturing of ID will be accomplished on separate schedules.

For OLD Students

  1. The student/parent must submit clearance, Form 9 (final Report Card), and the accomplished pre-registration form to the faculty-in-charge.
  2. The faculty-in-charge will check the completeness of the form.
  3. Once the form is correct and complete, signature/s will be affixed on the pre-assessment slip. Such a form will be given to the parent or student.
  4. The parent/student will pay the necessary fees at the Cashier’s Office. The parent or student will receive an official receipt to affirm the payment.
  5. The issuance of Certificates of Registration, Class Schedules, sections, and capturing of ID will be accomplished on separate schedules.
